Report a Lost/Stolen Passport to the Police

A U.S. passport is a valuable document, not just for you the rightful bearer. Therefore you must report the theft of a U.S. passport to the local police authorities to avoid Identity theft!  How can someone steal your identity? Identity theft occurs when someone uses your personal information such as your name, Social Security number, credit card number or other identifying information, without your permission to commit fraud or other crimes.

Under 16 Years

Please bring:

  • police report: If your passport was stolen, you should report it to the local police immediately and obtain a police report.
  • proof of citizenship; e.g. U.S. birth certificate, Consular Report of Birth Abroad, Naturalization Certificate,…
  • proof of identity: e.g. foreign passport, foreign ID-card, U.S. driver’s licence, copy of the most recent U.S. passport
  • one  U.S.-size photo (5 cm by 5 cm or 2″ by 2″) (see photo requirements and list of specialized photographers). Attention: for technical reasons no self-printed passport pictures can be accepted by the consulate! The passport photograph should comply with following guidelines:
    • One  unsigned photograph taken within the last six months.
    • 2×2 inch.(5×5 cm) square, with the image measured from chin to forehead not less than 1″ or more than 1-3/8″.
    • Photo should not be cut.
    • Color photos only will be taken in consideration; not tinted, with a white or off-white background.
  • fee: $115. Cash ($ and €) and credit cards are accepted. Visa and Master card holders are requested to obtain the authorization from their bank to enable payments in the U.S.
  • a completed printed online passport application form along with a statement regarding lost or stolen passport. (This form should be downloaded along with the DS-11 Application for Passport and brought with you when you go to apply for a replacement passport). When completing the Form DS-11, include the social security number (SSN). The application cannot be processed if the SSN is missing.
  • All minors age 16 and over must appear in person to sign their application. All minors under age 16 must also appear in person and their applications require the signature of both parents. More information on both parents presence and/or documentation requirement.

Over 16 years

Please bring:

  • police report: If your passport was stolen, you should report it to the local police immediately and obtain a police report, because when replacing a stolen passport, we need to send a police report to the Department of State in Washington, D.C.
  • proof of citizenship; e.g. U.S. birth certificate, Consular report of Birth, Naturalization Certificate,…
  • proof of identity: e.g. foreign passport, foreign ID-card, U.S. driver’s licence, copy of the most recent U.S. passport, cancelled US passport.
  • one U.S.-size photo (5 cm by 5 cm or 2″ by 2″) (see photo requirements and list of specialized photographers). Attention: for technical reasons no self-printed passport pictures can be accepted by the consulate! The passport photograph you want to submit to the Passport & Citizenship Unit for you to obtain your passport, should comply with following guidelines:
    • One unsigned photograph taken within the last six months.
    • 2×2 inch.(5×5 cm) square, with the image measured from chin to forehead not less than 1″ or more than 1-3/8″.
    • Photo should not be cut.
    • Color photos only will be taken in consideration; not tinted, with a light background.
  • fee: $145. Cash ($ and €) and credit cards are accepted. Visa and Master card holders are requested to obtain the authorization from their bank to enable payments in the U.S.
  • a completed printed online passport application form along with a statement regarding lost or stolen passport. (This form should be downloaded along with the DS-11 Application for Passport and brought with you when you go to apply for a replacement passport). The passport can be mailed to you by regular mail free of charge. Please be aware that these envelopes cannot be traced.  The Embassy declines any responsibility for the delivery of passports by mail.  If you prefer to have your passport returned to you via registered mail, please provide us with a stamp for registered mail and three stamps for regular mail at the time of your application.  You also have the option to pick-up the passport in person from the Consular Section during our public hours.
